Your new role:
We are seeking a dedicated Aboriginal and Torres Strait Islander Primary Health Care Practitioner to join our multidisciplinary team, delivering culturally safe and effective clinical care to the Metro South Aboriginal and Torres Strait Islander community affected by drug and alcohol challenges.

Key responsibilities include:
Providing comprehensive primary healthcare services within a multidisciplinary framework, tailored to the cultural and clinical needs of Aboriginal and Torres Strait Islander clients.
Managing a diverse caseload addressing mental health, substance use, trauma, grief, and social support needs through co-case management and care coordination.
Engaging effectively with individuals and families to support their navigation through the health system and ensure continuity of care.
Delivering health education, cultural support, and clinical guidance to both clients and health professionals to foster cultural competence and community involvement.

This is an identified vacancy with a genuine occupational requirement for the position to be filled by a candidate indigenous to the Aboriginal or Torres Strait Islander Community. Candidates not meeting this criteria will not be considered.

You will:
Have strong communication skills and the ability to build trustful relationships with Aboriginal and Torres Strait Islander individuals, families, and communities.
Demonstrate experience or a strong commitment to working within multidisciplinary teams focused on mental health and substance use care.
Possess cultural knowledge and the capacity to provide culturally safe care that aligns with the National Safety and Quality Health Service Standards.
