About the Opportunity
South Western Sydney Local Health District (SWSLHD) is seeking a dedicated and experienced Aftercare Lead to support the delivery and coordination of Aftercare services across the district.

In this role, you will work closely with the NSW Ministry of Health, Primary Health Networks, local mental health teams, and community organisations to embed and strengthen Aftercare pathways. You will provide expert advice, support governance and evaluation processes, and help ensure consistent, high-quality care for individuals following a suicidal crisis.

Key responsibilities include:

  • Coordinating stakeholder engagement and service integration

  • Supporting referral pathways and service improvements

  • Contributing to data reporting, evaluation, and quality initiatives

  • Promoting awareness and understanding of Aftercare services across the district

What You'll be Doing

The Aftercare Lead (the Lead) provides District-wide leadership for the integration of aftercare services including The Way Back NSW Aftercare Service in SWSLHD. The Way Back NSW provides community-based assertive psychosocial support for people following a suicide attempt or suicidal crisis who have been referred from an acute care or community mental health setting.

This role will lead the coordination of Aftercare services with relevant hospital and community mental health and suicide prevention services to ensure access to aftercare support is available for all people who need it.

The Lead will be required to plan and implement strategies to enhance service visibility for patients/consumers, families and clinicians, coordinate with District teams to ensure the effectiveness of referral pathways, and play a lead role in clinical and operational governance consistent with NSW Health and SWSLHD policies, procedures and standards.


Where You'll Be Working

SWSLHD Community Mental Health Services provide comprehensive, community-based mental health care across South Western Sydney.

These services cater to different age groups, including children, adolescents, adults, and older persons, ensuring specialised care throughout various life stages. The multidisciplinary teams offer a range of supports, including mental health assessments, crisis intervention, care coordination, and therapy. They work closely with inpatient services, GPs, and other community organisations to provide holistic care.

  • Tertiary qualifications and/or equivalent work experience relevant to mental health and suicide prevention.
  • Describe your experience working collaboratively with diverse stakeholders – including government agencies, health networks, and community service providers to implement or enhance mental health services. How have you contributed to embedding best practice approaches and improving referral pathways in a multidisciplinary setting?
  • How have you supported service design and implementation and contributed to service evaluation or data reporting in a mental health context? Please provide examples of how you’ve used data to inform service improvements, supported clinical decision-making, and promoted consistency in referral practices across a district or region.
  • Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
