Nepean Hospital is a teaching hospital that provides tertiary referral services in emergency care, coronary care, diagnostics, gynaecology, paediatric, neonatal intensive care/ICU, maternity, mental health, rehabilitation, and surgery. We are seeking a compassionate and skilled Social Worker to join our Aged Care team. In this role, you will deliver high-quality social work services informed by contemporary theories and practices, working closely with multidisciplinary teams to ensure the best outcomes for patients. You will conduct comprehensive psychosocial assessments, provide counselling across a range of needs including crisis, grief, and adjustment, and actively participate in case conferences and discharge planning. Your work will involve liaising with community agencies and services such as My Aged Care, Carers Gateway, NCAT Guardianship Division, and the NDIS to support complex referrals and transitions.

What you will bring to the role
- Tertiary Qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW)
- Demonstrated understanding of Aged Care Social Work within a Hospital environment
- Demonstrated high level communication, report writing and computer skills
- Demonstrated ability to work independently and as a part of a Multidisciplinary team.
- Demonstrated understanding of psychosocial stressors that could impact on the admission.
- Basic knowledge of My Aged Care, Carers Gateway, NSW Civil Administrative Tribunal - Guardianship Division, National Disability Insurance Scheme, the Ageing and Disability Commission and Residential Aged Care.
- Demonstrated capacity to Link theory and practice
- Current unrestricted driver's license and willingness to travel for work.
