About the Role

The purpose of the role is to ensure the best outcomes for our participants by navigating and facilitating their access to essential rehabilitation services and supports following discharge from hospital, and during their time of adjusting and settling into community life following serious injury. It involves establishing, coordinating and monitoring multi-disciplinary community-based rehabilitation programs with a focus on maximizing the participants’ recovery, independence and community engagement. This role requires travel to regional areas by car or aeroplane and may require overnight stays.

Duties

It’s an exciting time at icare where no two days are the same; as a Case Manager you will:

  • Manage a mixed caseload of participants accepted into the scheme including newly discharged post injury or in active community-based rehabilitation
  • Act as the single point of contact for those making applications to the scheme, liaising with key stakeholders to gather information to determine eligibility
  • Use a person-centred approach to support participants, developing rehabilitation plans that maximise their involvement through self-assessment, self-reflection and self-identification of strengths, priorities and goals.
  • Guide participants and their families in setting expectations and assist their understanding of what they can reasonably expect from Lifetime Care fostering a relationship built on trust.
  • Adopt a safeguarding and capacity building approach to ensure the safety and wellbeing of participants
  • Proactively work with participants, their social support networks, service providers, internal and external stakeholders to ensure services are delivered to achieve their identified goals through active participation in community-based rehabilitation programs
  • Provide timely, accurate and comprehensive information to enable participants to make informed decisions.
  • Apply critical reasoning and thinking within a decision-making framework when establishing and coordinating requests for services and supports to appropriately meet the treatment, rehabilitation and care needs of participants
  • Engage collaboratively with internal colleagues to make funding decisions regarding reasonable and necessary treatment, rehabilitation and care according to Guidelines within agreed functional delegations.
  • Discharge participants from case management when they no longer require it
  • Adhere to the KPI’s that monitor and measure key areas of the role.
  • Work collaboratively with internal teams to improve service delivery, identify gaps in services and ensure consistency in decision making.
  • Complete and maintain relevant records and essential documents and manage service approvals and payments in accordance organisational policies.
  • Regularly undertake metropolitan and regional travel to meet with participants in person, in their homes and in specialist hospital units to facilitate and support their discharge into the community following inpatient rehabilitation
  • Identify and participate in professional development opportunities to ensure currency of skills and knowledge with regard to current evidence and best practices
  • Keep up to date with industry trends, best practices and regulatory changes

Skills & Experience

  • Pre-Employment Checks
  • A current driver’s licence, with the ability to travel (may be by car or aeroplane).
  • Maintain a current Working with Children Check.
  • Up to date with current Public Health Order requirements
  • Demonstrate at least 5 years of full time (or equivalent) work experience which has equipped you with transferrable skills to provide case management of people with disabilities similar to those supported by our schemes and/or tertiary qualifications in an allied health, or other relevant, discipline.
  • Highly developed understanding of the needs and perspectives of people with disabilities.
  • Critical reasoning skills to ensure a high degree of integrity in the decision-making processes.
  • Flexibility and insightfulness and an ability to tailor established service delivery to meet the circumstances of each person
  • Ability to set clear expectations and engage in difficult conversations in a simple and empathetic manner.
  • A range of experience in coordinating multiple stakeholders supporting people and their families experiencing serious injuries and or significant and complex disabilities.
  • Established community networks in the health/disability sector.
  • Experience working with people with brain and/or spinal cord injuries.
  • Experience and understanding of working within legislative environments (eg Workers Compensation, Personal Injury or Disability)
