Full-Time NDIS Support Coordinator

As a NDIS Support Coordinator, you will play a vital role in assisting participants to navigate the National Disability Insurance Scheme (NDIS) and access the supports they need to achieve their goals. Your responsibilities will include:
· Conducting assessments to identify participant goals, strengths, and support needs.
· Developing and implementing individualised support plans in collaboration with participants and their families.
· Liaising with NDIS, service providers, and other stakeholders to coordinate supports and services.
· Providing information and guidance to participants on available services, funding options, and rights under the NDIS.
· Monitoring and reviewing participant progress towards goals and adjusting support plans as necessary.
· Advocating for participants to ensure their needs are met and rights are upheld.
Requirements:
· Experience working in the disability sector or a related field.
· Knowledge of the NDIS framework and principles.
· Strong communication and interpersonal skills.
· Ability to work independently and as part of a team.
· Excellent organizational and time management skills.
· Relevant qualifications in disability services or a related discipline (desirable).
· Lived experience with disability or experience working in a person-centred, strengths-based approach
· Knowledge of local service providers and community resources

Other Requirements
- Current Working with Children Check (WWCC) and National Police Check
- Valid driver’s licence and access to a reliable vehicle
- NDIS Worker Screening Check
