The Department
Caulfield Community Health Service provides community-based health and wellbeing services to people at all stages of life across Melbourne’s south-eastern suburbs. The Role
This Social Work role would suit an experienced, dynamic Social Worker with a passion for community-based clinical practice. The Social Worker works collaboratively as part of a multi-disciplinary team, and the referrals that make up the clinical caseload will be varied.
The clients our team works with will mostly be over 65 and may may be facing hardship, have practical concerns, health concerns, challenging family dynamics or having difficulty accessing service systems. The Social Worker would see clients either at the community health centre, or in their own homes.
Qualifications and Experience Required
You will be an integral member of the multi-disciplinary team, and will work at times autonomously and always collaboratively to ensure the needs of clients and carers are addressed.
You will:
- Provide advanced Social Work services for older Adult clients including: psychosocial assessment, counselling, interim case management and casework
- Offer individual and group work, advocacy, and outreach
- Demonstrate an understanding and experience in navigating pathways within the health care system (e.g. My Aged Care, NDIS, Commonwealth Home Support Program, Home Care Packages, VCAT)
- Understand issues affecting clients experiencing Elder Abuse or Family Violence and a be able to respond effectively to these issues
- Have a Tertiary qualification in Social Work with eligibility for membership of the Australian Association of Social Workers
- Demonstrate Social Work skills and experience at a Grade 2 level, and have at least three years’ post qualifying experience
- Have a current Victorian Driver’s Licence
