About the Role:
The Northern Adelaide Local Health Network is seeking a dedicated Social Worker to join our Northern Adelaide Geriatrics Service, playing a pivotal role in supporting our vulnerable community members.

In this role, you'll work within our dedicated Geriatric Evaluation Management unit, providing comprehensive psychosocial assessments and interventions for older adults. This position offers the perfect work-life balance whilst delivering meaningful healthcare outcomes.
Key responsibilities:
- conduct thorough psychosocial assessments for elderly patients
- develop and implement discharge planning strategies
- collaborate with multidisciplinary teams including geriatricians, nurses, and allied health professionals
- provide counselling and support to patients and their families
- facilitate community referrals and coordinate ongoing care services
- advocate for patient rights and optimal care outcomes
To be eligible for this position, you must hold an appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
