We have an exciting opportunity for experienced and passionate Support Coordinators to join a team in our Universal Aftercare program - Care Connect, across the Greater Newcastle, Lake Macquarie, Port Stephens & Lower Hunter regions.

Care Connect is an aftercare service supporting people experiencing a suicidal crisis or who have attempted suicide. The service focuses on empowering people and their care givers to connect with formal and practical support to assist with recovery. Permanent part-time position of 4 days per week.

Job Description

Your role within our dedicated suicide aftercare program, Care Connect, will allow you to:

  • Provide therapeutic supports and short-term psychological interventions, connecting participant to services and supports
  • Expand your case management and care coordination skills
  • Be a part of a multidisciplinary team in providing in evidence-based best practice and high quality services
  • Build partnerships with other community services and expand your network

We are looking for passionate individuals with a desire to make a difference. The successful candidates will have:

  • At least Diploma or Cert IV level qualification in Mental Health or Community Services
  • Two years’ experience in a suicide prevention / aftercare or mental health setting
  • Previous experience in providing case management to vulnerable groups
  • Demonstrated experience in maintaining professional boundaries while engaging in person-centred work
