Our North Queensland office continues to grow and we are looking for an experienced and dedicated Care Coordinator to support this continued growth in the Charters Towers Region.
Role Overview
The successful candidates will preferably have a strong background in Community Aged/Disability Care. The role itself is responsible for service development, coordination and delivery, including ongoing client assessments and development of care plans that deliver flexible and personalised in-home support. Our ideal candidate will be either a qualified EN or RN with a current AHPRA-NUrse registration in order to provide direct nursing support to clients is also incorporated within the role, although a certificate IV in aged care will be acceptable for the right candidate. Successful applicants will thrive on working in a close-knit team and have a passion for delivering the highest quality in client care. This role is a full time role based in Charters Towers, however part time may be considered for the right candidate.
This key role will focus on case management, nursing support, customer service, personal care work where required and general administration.
Major duties include:
- Care Assessments;
- Government-Compliant Care Planning;
- Building and maintaining customer relationships;
- Quality assurance; and
- Nursing supports (EN/RN only).
Qualifications
- Minimum of Certificate IV Aged Care;
- Diploma/Bachelor in Nursing with a current AHPRA registration highly regarded;
- Driver's License and comprehensively insured dependable private vehicle;
- Current National Police Clearance; and
- Current First Aid & CPR.
Preferred Skills;
- Good understanding of the Government funding environment for aged care (primarily CDC) & NDIS (preferred but not necessary);
- Good understanding of the hospital discharge environment;

- Excellent communication abilities, professionalism, a good reputation & respect for our staff, clients and their families; and
- It is also essential to have good judgment, quick decision-making, excellent communication, staff management and administrative skills.

Pay: $75,000.00 – $85,000.00 per year
