The Care Partner holds direct responsibility for managing an assigned caseload and will be actively engaged in all aspects of care planning, service coordination, monitoring, review, and evaluation of services. Additionally, the role includes documentation, risk assessments, clinical governance, and quality improvement initiatives. The Care Partner will also oversee client budgets, ensuring services are delivered within the financial constraints and in alignment with the client's needs, preferences, and rights, particularly with respect to choices, dignity, privacy, and respect.
About you
We want to hear from you if you have;
- Experienced in Aged Care Sector/ Home care and community sector
- Case Management, Caring, Compassionate and Responsible
- Demonstrated experience or genuine interest in supporting older people to live independently and with dignity in their own homes.
- Ability to build trusting relationships with clients, families, and team members through respectful and empathetic communication.
- Reliable, self-motivated, and capable of working independently while following care plans and safety procedures
