About the role

As a dedicated member of the HARP Diabetes Team you will provide expert care to patients living with diabetes. who may find self-management challenging. You will be a part of a multidisciplinary team with lots of potential for professional growth. A current drivers licence is essential as you may be required to attend home visits from time to time. A working knowledge of CGM will be highly regarded.


Key responsibilities:

  • Support patients with diabetes attending outpatients clinics at WH
  • Support patients with diabetes undergoing dialysis
  • Provide self-management education to patients living with diabetes or their carers

About you:

  • You will be a credentialed diabetes Clinical Nurse Consultant with a tertiary qualification in diabetes self-management
  • You will be dedicated to improving outcomes of people living with diabetes
  • You will be a team player who can work automomously if required
