Discharge Planning Co-ordinator

• Do you have current registration with Nursing & Midwifery Board of Australia as a Registered Nurse (Division 1) with relevant clinical experience to validate knowledge of both inpatient clinical services and community-based services.

• Do you have demonstrated working knowledge of services both within the inpatient and community health setting including My Aged Care, ACAT, GPs, Community Health and chronic disease management.
