About Us:
Triniti Home Care is a leading provider of NDIS services, dedicated to empowering people with disabilities to live fulfilling and independent lives. We are currently seeking two full-time Support Coordinators to join our passionate team—one in the Newcastle–Gosford area and one in the Campbelltown–Macarthur region. These roles offer a hybrid working model, combining work from home with community-based visits.
About the Role:
As a Support Coordinator at Triniti Home Care, you will assist NDIS participants in navigating the system, accessing appropriate supports, and achieving their goals. You will play a key role in ensuring participants feel empowered, respected, and well-supported throughout their journey.

Key Responsibilities:
- Support participants to understand and implement their NDIS plans.
- Connect participants with services and supports that meet their individual needs.
- Monitor progress, update plans as required, and ensure services remain aligned with goals.
- Build strong, respectful relationships with participants, families, and service providers.
- Keep accurate, compliant, and up-to-date records.
- Advocate for participants to ensure their choices and voices are central to support decisions.
What We’re Looking For:
- A genuine passion for supporting individuals with disabilities.
- Strong communication, organisation, and time-management skills.
- Ability to work independently and within a team.
- A current driver’s licence and access to a reliable vehicle.
- Relevant qualifications in Social Work, Disability, or a related field are desirable but not essential.
- Previous experience in support coordination or the disability sector is valued but not required.

Pay: $32.00 – $42.00 per hour
