We are seeking a full-time Care Partner to join our Home and Community Services team. This requires travel to client’s home however office-based tasks can be completed remotely (WFH). As a Care Partner you will play a critical role in delivering person-focused, high-quality home care services through assessment, care planning, monitoring, and review.
Reporting to the Clinical Care Manager, you will work closely with clients, families, and internal teams to ensure services meet individual needs and align with McLean Care’s values and quality standards.
Key Responsibilities
Conduct client assessments and develop individualised care plans
Coordinate and monitor service delivery to ensure quality and compliance
Document and maintain accurate client records
Collaborate with rostering and administrative teams for smooth service delivery
Advocate for client wellbeing while respecting confidentiality and dignity
Identify additional support needs and facilitate referrals where appropriate
Participate in team meetings, WHS training, and quality assurance processes
Contribute to service development and community engagement
Certificate IV in Ageing Support, or
Enrolled Nurse (Diploma of Nursing), or
Registered Nurse (Bachelor of Nursing)
Current First Aid and CPR certification
Unrestricted driver’s licence and own insured, registered vehicle
Experience in assessment, referral, and service coordination within aged or community care
Ability to balance client needs and program resources effectively
High-level interpersonal and communication skills
Proficient in care documentation and use of client management systems
Ability to work independently and collaboratively within a team
Understanding of WHS, Duty of Care, and the rights of clients in aged care
Commitment to person-centred care and McLean Care’s organisational values
Knowledge of Commonwealth Home Support Programme (CHSP) and Consumer Directed Care
Familiarity with Aged Care Quality Standards
Experience in coordinating services for older people in the community
Competitive hourly rate of $41.41 - $56.27 per hour (dependent on qualification) + super + allowances
