Home Care Coordinator

About the role
We have an exciting opportunity for a passionate and driven Home Care Coordinator to join our team in the South East Metro area. In this rewarding role, you will provide dedicated support to clients receiving Home Care Packages (HCP) and services under the Commonwealth Home Support Programme (CHSP).
Working closely with the Senior Care Team Coordinator, you will play a key role in supporting the coordination of both staff and client services, helping to ensure high-quality, person-centred care.
About you
You’re a compassionate and confident leader with a deep understanding of aged care services and a strong commitment to person-centred care. You bring a collaborative and empathetic approach to your work, building meaningful relationships with clients, families, and colleagues. Your supportive and inclusive leadership style empowers older Australians to live well and independently in their own homes.
In addition, you will also possess:
- Certificate IV in Ageing or equivalent is highly desired
- Experience in community, aged care or support coordination, with a solid understanding of person-centred care.
- Strong organizational and time management skills
- Strong understanding of aged care standards and compliance
- Current first aid certificate or willingness to obtain
- Drivers Licence
- National Police Clearance (obtained within the last 6 months)
