About the Role
As a Lifestyle Facilitator (Support Worker), you will deliver tailored support services that promote independence and inclusion for people living with a disability. Your work will focus on building life skills across social, recreational, employment, and daily living activities.
You will play a vital role in encouraging community participation, personal choice, and meaningful engagement – always with a person-centred approach.

Skills & Experience
- Experience working with individuals with disabilities or mental health conditions
- Comfortable supporting Participants with complex needs and behaviours
- Ability to communicate effectively with Participants, colleagues, and stakeholders
- Experience in active overnight shifts and manual handling tasks
- Strong mentoring and coaching abilities
Required Certifications
- QLD Green Provisional or Open Driver's License & comprehensively insured vehicle
- Current Blue Card (Working with Children Check)
- NDIS Worker Screening Check (Yellow Card)
- Completion of the NDIS Worker Orientation Module
- First Aid & CPR certification
- Proof of two doses of COVID-19 vaccination
Desirable (Not Essential)
- Experience in Disability or Community Services
- Prior work in Supported Independent Living (SIL)
- Familiarity with Positive Behaviour Support Plans
- Understanding of privacy and confidentiality principles
- Ability to build and maintain strong networks with external service providers
Key Responsibilities
- Support Participants in achieving goals outlined in their NDIS Plans
- Promote positive behaviour and implement relevant support strategies
- Assist with personal care tasks such as showering, toileting, and dressing
- Perform manual handling (e.g., hoist transfers, use of wheelchairs)
- Facilitate social and community engagement
- Support household tasks like meal prep, tidying, and basic cleaning
- Work closely with Participants and their support networks
- Follow all abbaCare policies, procedures, and relevant industry standards
