Position Overview
As a Care Coordinator, you will play a crucial role in ensuring the effective delivery of comprehensive care services to individuals with disabilities supported by Care World. Your primary responsibilities will involve overseeing and coordinating support services for participants, who are funded under the NDIS, iCare, or who are self-funded.
The Care Coordinator role involves client engagement, care planning, compliance, team collaboration, conflict resolution, stakeholder consultation, mentoring and advocacy.

Main Responsibilities
- Work closely with a multidisciplinary team, including healthcare professionals, support workers, and other stakeholders, to develop and implement person-centered care plans.
- Act as a liaison between the organisation, NDIS participants, their families (where consent is provided), support workers and other stakeholders.
- Understand and address the unique needs, preferences, and goals of individuals with disabilities.
- Assess, plan, and organize a range of support services, such as personal care, therapy sessions, and community engagement activities, to enhance the overall well-being and independence of NDIS participants.
- Stay informed about NDIS guidelines, regulations, and funding requirements. Ensure that provided services align with the NDIS framework.
- Coordinate assessments, reviews, and ongoing monitoring of participants' progress. Advocate for necessary adjustments to care plans based on individual needs.
Duties & Responsibilities
- Engaging new clients and assisting them to set up their funding for Care.
- Management, supervision, providing support and mentoring to Care Staff
- Create Care Plans so it meets customer short term and long-term goals.
- Assisting customers to navigate their care plans and setting up services with our partner providers - to support customers to reach their wellness goals.
- Monitoring and reviewing customer care plans to ensure their ongoing needs are met.
- Developing strong relationships with our partner networks and other agencies.
- Maintaining accurate records of interventions, service schedules and customer care plans.

- Primary point of contact for our Support Workers and will be responsible for recruitment,
supporting and training our carers.
- Work with Admin Manager to roster appropriate care staff to our clients to ensure we
are delivering exceptional customer service
- Undertake risk assessments associated with the delivery of services
- Answer new client enquiries over the phone in a knowledgeable manner with the aim
of scheduling a care consultation
- Coordinate the provision of quality person centered care according to the client's
needs, service/care plans and available funding in accordance with budget
- General administration with respect to tasks performed
- Work on continuous improvement plan & Risk register
KPIs
- Measure the level of satisfaction among NDIS participants with the care services
provided, through regular feedback surveys or interviews.
- Monitor the adherence of support workers and care services to the established care
plans for participants, ensuring a high rate of compliance.
- Ensure the timely establishment of services and supports for new participants.
Experience/ Qualifications
- Minimum 1 year’s experience as a Care or Support Coordinator for NDIS Participants
preferred
- Qualifications in a relevant health-related field is highly desirable
- Knowledge of NDIS Practice Standards
- Excellent documentation skills.
- Computer literate and competent in the use of various platforms and apps.
- Cultural Competency to ensure appropriate services.
- Demonstrate knowledge of NDIS Supports.
- Police Check
- Working with Children Check
- Own registered vehicle and current drivers licence.
