What you'll be doing
Pregnancy Family Meetings are a collaborative frontline program between Department of Communities & Justice Western Sydney Nepean /Blue Mountains District (DCJ) and Western Sydney Local Health District (WSLHD) that uses family meetings to promote early engagement and interagency planning with pregnant women at risk of their babies entering out of home care at birth. The program is voluntary and offered to pregnant women who are linked in with DCJ who meet ROSH (Risk of Significant Harm) and who are also having their antenatal and birthing care through one of WSLHD’s Hospitals.
Pregnancy Family Meetings (PFMs) are facilitated by facilitators who are independent of case management responsibilities, decision making about infant care outcomes and any clinical involvement with the family. This is critical to ensure the mother feels safe and comfortable.
The Pregnancy Meeting Facilitator is a key facilitator position in the PFM Program. The position reports to the Pregnancy Family Meeting Coordinator for WSLHD and is responsible for:
- Working directly with the family and agencies to schedule and then facilitate Pregnancy Family Meetings.
- Supporting the PFM Coordinator to develop and maintain a group of appropriately skilled PFM Facilitators from within WSLHD
- Developing and fostering strong interagency partnerships to sustain and further develop the PFM Program
