Your Opportunity
HASI is a community mental health recovery program funded by Ministry of Health. The service works in partnership with relevant stakeholders in particular with the community mental health service. The program provides recovery based psycho-social support with the client in the centre. Entails a combination of case work and support work. Most of the role is direct frontlineservice delivery; face to face with client in the community. Support workers mainly support the team with various clients in the community. Case workers do a combination of case coordination and support work.
We are seeking a compassionate and motivated Recovery Case Worker to join our team and deliver client-centred support that empowers individuals on their recovery journey. Working within a recovery-oriented and trauma-informed framework, you will support clients to build independence, wellbeing and community connection.
This is a max term position with the possibility of extension. Flexible work arrangements are available, and we welcome applications from candidates seeking part-time or casual employment.
Your key responsibilities will be to:
- Provide culturally responsive, person-centred case management in line with Mission Australia’s Recovery and National Case Management frameworks
- Conduct client intake, assessments, registrations and develop strengths-based case plans
- Deliver ongoing outreach support to assist clients with day-to-day living skills, medication observation and service navigation
- Monitor and review client progress towards goals, assisting with transition to independence or onward referrals
- Support delivery of group programs and life-skills training under direction
- Build effective working relationships with internal teams, service providers, government and community stakeholders
- Complete required administrative and compliance duties, ensuring accurate case notes, data entry and reporting
Requirements for Success
- Relevant tertiary qualification or equivalent experience in the community services, social work and/or mental health space
- Experience in case management or support work within community, mental health, homelessness or related services
- Knowledge of recovery-oriented practice and ability to work collaboratively with clients to achieve their goals
- Strong interpersonal and communication skills; able to build trust and rapport
- Ability to work autonomously under general supervision and manage competing priorities
- Committed to Mission Australia’s values and strengths-based practice
- Driver’s licence with no restrictions
