As a Support Coordinator, you will play an integral role in supporting participants to achieve their goals in accordance with their NDIS plans. This includes providing ongoing support coordination services, engaging with participants, and collaborating with their supports and third-party organizations in a professional manner.
Key Responsibilities:
- Work with participants to achieve their goals in line with their NDIS plan.
- Provide ongoing support and use a coaching approach to assist in capacity building.
- Maintain accurate and up-to-date case notes via our CMS.
- Ensure adherence to Work Health and Safety guidelines.
- Collaborate in a professional and collaborative manner with all involved parties.
- Undertake work in line with the organization's policies, procedures, and values.
- Comply with all relevant legislation and code of conduct requirements.
- Maintain clear professional boundaries with participants and up-to-date knowledge of the NDIS system, local services, and organizations.
Qualifications and Skills Required:
- Minimum 6 months to 2 years experience working as an NDIS Support Coordinator or in a similar role.
- Allied Health qualifications; or minimum Certificate IV in Allied Health/Disability/Aged Care or a diploma/university degree in a related field.
- Experience or knowledge in handling complex family health issues would be beneficial.
- Excellent communication and interpersonal skills.
- Demonstrated ability to work independently and as part of a team.
