As a Support Worker, you will provide personalised support to people with disabilities in their homes and communities, promoting independence, choice, and wellbeing. This role involves assisting clients with daily living tasks, social participation, and personal goals, while maintaining professional boundaries and upholding Proactive Supports 4 U values.
- Provide support with daily living activities such as personal care, meal preparation, and household tasks
- Assist with community access and social participation (e.g. shopping, appointments, recreational activities)
- Promote independence and client choice in all tasks
- Maintain accurate and timely progress notes and communication logs
- Follow client care plans and behaviour support plans
- Ensure client privacy, dignity, and safety at all times
- Work within Proactive Supports 4 U policies and procedures
- Report concerns, incidents, or changes in client needs to relevant staff
- Assist with medication prompting or administration (if required and trained)
- Participate in team meetings, training, and supervision sessions
Key Requirements:
- Current NDIS Worker Screening Check
- Current Working With Children Check
- Current Level 2 First Aid and CPR
- Current Police Check
- Reliable, roadworthy vehicle with full comprehensive insurance (if transporting clients)
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Respectful, person-centred approach to support
- Willingness to work flexible hours, including evenings or weekends
