Triniti Home Care is a leading provider of NDIS services, dedicated to empowering people with disabilities to live fulfilling and independent lives. We are currently seeking a full-time Support Coordinator to join our passionate team, based in Greater Melbourne & Western Suburbs (servicing all suburbs including Werribee, Watergardens, St Albans, Noble Park, and Sunbury). This role offers a hybrid working model, combining work from home with community-based visits.

This role also offers the opportunity to grow into Management positions for suitable candidates, with salary negotiable based on experience and capability.
About the Role
As a Support Coordinator at Triniti Home Care, you will assist NDIS participants in navigating the system, accessing appropriate supports, and achieving their goals. You will play a key role in ensuring participants feel empowered, respected, and well-supported throughout their journey.

Key Responsibilities
- Support participants to understand and implement their NDIS plans.
- Connect participants with services and supports that meet their individual needs.
- Monitor progress, update plans as required, and ensure services remain aligned with goals.
- Build strong, respectful relationships with participants, families, and service providers.
- Keep accurate, compliant, and up-to-date records.
- Advocate for participants to ensure their choices and voices are central to support decisions.
What We’re Looking For
- A genuine passion for supporting individuals with disabilities.
- Strong communication, organisation, and time-management skills.
- Ability to work independently and within a team.
- A current driver’s licence and access to a reliable vehicle.
- Relevant qualifications in Social Work, Disability, or a related field are desirable but not essential.
- Previous experience in support coordination or the disability sector is valued but not required.
- Strong connection and network in the community, with the ability to engage and have existing clientele.

Pay: $32.00 – $42.00 per hour
