Make a Real Difference in the Lives of Individuals Experiencing Homelessness

The Salvation Army invites you to be part of a life-changing mission through our Flagstaff and The Open Door residential services – offering short-term accommodation and holistic support for single men experiencing homelessness and facing multiple, complex challenges.
About You
We’re looking for someone with a strong sense of purpose and experience in the social services sector — a confident practitioner who leads with empathy and believes in people’s ability to change and grow.
About the Role
As a Transitional Housing Case Manager, you’ll be a vital part of a collaborative team working to empower men on their journey toward long-term, dignified housing. You’ll walk alongside each individual, offering consistent, trauma-informed, person-centred support that builds resilience, independence, and hope.
You'll support residents through individualised case management and outreach, ensuring a warm and coordinated transition from crisis accommodation to sustainable housing options. Your work will be grounded in compassion, respect, and cultural sensitivity — ensuring every man is seen, heard, and valued.
Reporting to the Residential Case Management Services Team Leader, this is a permanent full-time position, based in Flagstaff In West Melbourne and The Open Door in North Melbourne, VIC.
Compensation is in accordance with SCHADS Award Level 5.
You Will Successfully
- Develop and implement strengths-based, solution-focused case plans in partnership with clients
- Support transitions to long-term housing through warm referrals and coordinated outreach
- Advocate across housing, health, legal, and community systems to meet client needs
- Collaborate with internal teams and external partners to connect clients with relevant services
- Maintain accurate records and contribute to service development and continuous improvement
You Will Have
- A degree qualification in Social Work, Psychology, or a related field
- Demonstrated experience in case management within a homelessness or community services setting
- Knowledge of homelessness, trauma, and housing systems (including the Victorian Housing Register)
- Strong communication and advocacy skills
- A collaborative spirit and ability to build positive relationships with clients and stakeholders
- Cultural awareness and a commitment to inclusive practice
- A deep alignment with The Salvation Army's mission, values, and commitment to justice
- Current Australian Driver’s Licence
- An Employee Working with Children's Check
