Primary objective
Conduct comprehensive and home support assessments for aged clients referred to the Aged Care Assessment Service in
Alice Springs and remote areas in Central Australia.
Key duties and responsibilities
1. Undertake sponsored training in Commonwealth aged care assessment if necessary.
2. Undertake comprehensive and home support assessments, consultation, education and intervention for aged people
across Central Australia and Barkly regions.
3. Ensure effective continuity of care through case coordination of clients with complex needs, working collaboratively
with the
4. multidisciplinary team and relevant stakeholders.
5. Ensure that high quality services are delivered and professional clinical skills maintained and developed by proactively
6. participating in clinical supervision, performance management and professional development.
7. Comply with Department of Health and Commonwealth Aged Care Assessment Service guidelines and systems.
8. Follow defined service quality standards, work health and safety policies and procedures relating to the work being
undertaken in order to ensure high quality, safe services and workplaces.
Selection criteria
Essential
1. Professional qualifications as an Occupational Therapist, Physiotherapist, Social Worker, Speech Pathologist, Nurse or
related field with current registration where appropriate.
2. Demonstrated post-graduate experience providing holistic, clinical assessment and intervention to the aged population.
3. Demonstrated experience working effectively within a multi-disciplinary team, including the ability to prioritise and
manage caseloads in an efficient and timely manner.
4. Demonstrated high quality oral and written communication skills, computer skills, report writing and maintenance of
clinical records.
5. Proven skills in problem solving, negotiation and conflict resolution which reflect and value client participation and
community inclusiveness.
6. Demonstrated ability to act as a consultant to a wide range of organisations, service providers and individuals and work
experience developing and facilitating health education training programs.
7. Ability to travel to remote regions and NT Driver’s C Class Licence or ability to obtain.
Desirable
1. Successful completion of Aged Care Assessment Service Training.
2. Post graduate qualifications/studies in aged care.
