About the Role

AHP-1 As an Social Worker, you’ll have the opportunity to make a real difference in the lives of patients and their families by supporting their psycho-social wellbeing through expert assessment, counselling, and collaboration. Working within a supportive multidisciplinary team across CALHN’s Single Service, Multiple Site model, you’ll be actively involved in clinical activities, data reporting, and service planning. You’ll also engage with community resources to ensure holistic care and positive outcomes. This is a great role for a motivated early-career professional eager to grow their skills while contributing to quality improvement and research initiatives.

AHP-2 As an Social Worker, you’ll play a vital role in supporting patients and families to adjust to life’s challenges by applying your expertise in counselling, advocacy and psycho-social care. You'll work closely within a multidisciplinary team, actively participating in ward rounds and clinical meetings while also contributing to data reporting and service evaluation. Based within the CALHN Single Service, Multiple Site model, you'll connect people with essential community and organisational resources, ensuring holistic care. This is a rewarding opportunity to shape outcomes, support best practice, and be part of innovative quality improvement and research initiatives.
