Key Responsibilities:
As a Care Coordinator, you will:
- Development, implementation, monitoring and review of care plans.
- Updating Care Plans and Budgets for our clients to ensure that their care needs and deterioration of health are well documented.
- Meeting compliance targets for all mandatory documentation including time management strategies to ensure community and office time.
- Adhere to the NDIS Commission Standards and Aged Care Quality Standards;
- Monitor WHS and equipment in client's home and review based on a risk management process.
- Document all correspondence with clients, client representative and stakeholder in clients file in profession unbiased manner.
- Respond and follow feedback through app notes, complaint and incident procedures creating a continuous improvement environment
- Ensure holistic service delivery by comprehensive case management , goals development, carer intros, supervisory visits and documentation of all client monitoring.
- Prioritizing and managing a high volume of inbound and outbound calls.
- Acting as the primary point of contact for our clients regarding any incidents and guidelines regarding the service.
- Encourage teamwork and foster good communication with staff, clients, relatives, medical practitioners, allied health professionals and the community which the program services.
- Provide or refer to clinical team for ongoing clinical advice and guidance.
- Ensure clear scope of practice and indivdualised care planning documentation and guidance for care staff to complete services-based on client goals
- Understand special needs groups and respond in cultural inclusive manner with all care planning tasks
- Coordinate the provision of quality person centered care according to the client's needs, service/care plans and available funding in accordance with budget
- Refer all clients to appropriate health professions and adhere to third party vendor procedures
- Have a comprehensive understanding of funding model, expense options and ability to resolve budget considerations with client.
- Ensuring compliance and legislation is adhered to at all times.
- Compile and prepare reports for management and other external stakeholders.
Qualifications and Skills:
- Certificate III in Aged Care / Individual Support / Community Care (or higher qualification in relevant discipline).
- Knowledge of the aged care industry and related issues.
- Proven ability to assess, plan, implement, and evaluate care services.
- Strong interpersonal and communication skills, both written and oral.
- Computer literacy and analytical abilities.
- Commitment to the Aged Care Sector and the well-being of seniors and vulnerable individuals.
- Driver's License and comprehensively insured private vehicle.

Pay: $70,000.00 – $90,000.00 per year
