About the role:

Join our Intensive Therapeutic Care (ITC) team as a Case Manager to provide tailored, high quality and effective case management to vulnerable children and young people in our program. We work in collaboration with clients, internal staff and service providers to assess needs, develop care plans and monitor progress to ensure the goals of children and young people is being met. This is a permanent, full-time role based on the Central Coast.
This role sits within our Intensive Therapeutic Care (ITC) and Therapeutic Supported Independent Living (TSIL) program, which provides an alternative to long term residential care, focusing on recovery from trauma, the achievement of positive life outcomes and a broad range of placement options for children and young people.
CatholicCare provides flexible working options and various learning & development opportunities. Flexibility to work some additional hours, including at weekends, and to undertake on-call duties is required for this role from time to time. Reporting to a Manager, Casework, you will be supported by an experienced team of passionate people working together to change the lives of young people and promote change in the Community.
Main duties:
- Case planning and holistic assessment of young people to promote and support their safety, well-being and permanency needs.
- Working closely with young people and their families to develop pathways for the young people to transition through care and ultimately leave care.
- Timely completion of case notes, panel reports and case work reports.
- Developing positive relationships with young people and their families and stakeholders.
- Making permanency referrals for after care, leaving care and TSIL/SIL.
- Degree qualifications in Social Work, Psychology (or equivalent) and ideally have additional training or qualifications in developmental trauma, child protection, client intervention and case management.
- Be able to demonstrate case work experience working with children and young people who have experienced complex trauma.
- Have experience working with families who have been in contact with the child protection system.
- A strong understanding of the Intensive Therapeutic Care model.
- Have an unrestricted driver’s licence & own car.
- Ongoing reflective/practice / clinical supervision provided as part of the role.
