People’s First Choice Australia is a well-established accredited provider of NDIS Disability Services in Victoria assisting those with disabilities. We provide a wide range of in-home, respite, community, SIL and nursing support services to adults and children with disabilities and other challenges caused by illness and/or medical conditions.

Main Responsibilities
You will:
· Network and liaise with key Hospitals, Support Coordinators, and other relevant stakeholders.
· Resolve any issues that arise to ensure clients are satisfied with our services. In this role, you should be an excellent communicator who’s able to grasp customer needs and brainstorm ways to fulfil them.
· Have the central intake responsibility for NDIS participants and for ensuring the provision of safe and effective services and supports for daily living that optimise the client’s independence, health, well-being, and quality of life.
· Driving business development and related activity that supports the business’ growth aspirations of the organisation.
· Undertaking activities to grow enquires which may include expos, networking events, presentations to referrer groups and meetings with potential new referrers.
· Exploring and implementing new approaches to improve client experience
· Actively managing client intake and transition
· Ensuring the delivery of high-quality service including reporting, documentation and other duties as needed.
· Representing People’s First Choice Australia in a positive and responsive manner
In this role, you will also be responsible for:
· Completing Participant Support Plans that reflect participant aspirations, respond to their current service needs and will contribute to their overall health and wellbeing.
· Focusing on finding the service solutions that maintain and strengthen individual participant capacity and independence so they can continue with their activities of daily living and retain their social, community and family connections.
· Providing the Service Delivery team with completed intake details that allow for the timely commencement of service(s).
· Responding to families with queries and concerns
· Scheduling appointments and meetings
· Escalating concerns with PFCA
Requirements:
· A minimum of 6 months experience working in the health and disability industry is preferred
· Commitment to the provision of exceptional customer service
· Passionate about the work and have patience and mentality to deal with disabled people specially who are mentally challenged
· Well-developed interpersonal, negotiation and business development skills, particularly the promotion of current services and connecting with local communities and referral networks
· Excellent organisational and time management skills with the ability to prioritize and manage multiple and competing work tasks and deliver to agreed deadlines
· Advanced computer skills, including the use of Microsoft Office and a range of online systems.
· Unrestricted Driving Licence. Own car.
Job Types: Part-time, Casual
Pay: $60,086.40 – $72,323.20 per year
