Clinical Aged Care Needs Assessor
Join a supportive team environment and make a difference in older people’s lives. As a clinical assessor, you will be travelling to older peoples’ homes to conduct a clinical assessment. You will help them by connecting them to the services they require to stay independent and healthy for longer.

Your responsibilities:
- Travel to complete face to face assessments within Northern and South East Sydney and use your clinical expertise to assess the needs of older Australians
- Undertaking assessments with a strength-based approach, with a good understanding of Wellness, Reablement and increasing independence for older Australians.
- Conducting client reviews to keep up to date with client’s changing needs and circumstances
- Completing client assessments and support plans within the deadlines provided
- Connecting the client with service providers and community support organisations local to them
To be successful for this role you must:
- Be a registered nurse or allied health professional such as occupational therapist, social worker, physiotherapist.
- Have current public liability insurance
- Hold a current certificate in aged care Clinical Assessor qualification OR willingness to complete this training prior to delivering Regional Assessment Services.
- Have a good understanding of My Aged Care and the Integrated Assessment Tool (IAT) or willingness to learn.
- Have detailed knowledge of My Aged Care, related aged care programs and services with proven ability to use this knowledge to assess and support both routine and complex aged care presentations
- High level computer literacy, including proficiency of typing and MS Office programs.
- Have your own car and prepared to travel to deliver face to face assessment services in the client’s own home
- Hold a full current driving licence
- Hold a current AHPRA registrationr relevant registration.
Desirable skill set includes:
- Experience in assessment and goal orientated support planning in aged, community, health or social services, and previous experience working in assessment and/or coordination of services
- Confident communication and interpersonal skills to engage and motivate clients, families, staff, and other stakeholders with a positive approach
- Proactive, self-motivated person with the capacity to follow due process.
- Good team member, with excellent communication skills
- Second language is a distinct advantage
- Ability to work on online platforms such as My Aged Care portal.
Job Type: Casual
Pay: $35.00 – $40.00 per hour
Expected hours: 14 – 16 per week
