We’re currently seeking a Permanent Part-Time Clinical Nurse Specialist to join our dedicated team. This role is ideal for an experienced nurse who is ready to take on a broader clinical scope and provide advanced care and guidance in a home care setting.
About the Role
As a Clinical Nurse Specialist, you will apply advanced clinical skills to support complex client needs, contribute to best-practice care planning, and act as a key resource for other staff. This role blends hands-on care, clinical guidance, and continuous improvement in practice.
You must be available to travel across the South Brisbane region to deliver care to clients’ homes with a minimum of 12-14 hours/week.
What You’ll Be Doing
Advanced Clinical Practice
- Complete comprehensive client assessments and develop tailored care plans
- Safely administer medications and treatments in accordance with legislation and policy
- Monitor client health and respond to signs of deterioration
- Maintain clear, timely, and accurate clinical documentation
Clinical Support & Guidance
- Provide mentorship and day-to-day clinical guidance to other staff
- Assist with clinical decision-making and ensure care delivery aligns with standards
- Lead shift handovers or team briefings when needed
- Offer feedback and promote safe, accountable care practices
Communication & Collaboration
- Liaise with families, GPs, and allied health professionals
- Attend family meetings and case conferences when appropriate
- Document discussions and clinical decisions clearly in care records
Quality, Safety & Development
- Participate in audits, risk reporting, and improvement activities
- Comply with infection control, WHS, and clinical governance standards
- Stay up to date with best practices through ongoing learning and reflection
What You’ll Need
- Current AHPRA registration as a Registered Nurse
- Minimum 2 years’ post-graduate clinical experience
- Experience in community nursing
- Strong clinical judgment and critical thinking skills
- A collaborative approach and desire to support others
- Commitment to high standards of care and continuous improvement
- Willingness to work across all shifts
- A valid driver’s licence and reliable transport
- Availability to travel across South Brisbane
- A current National Police Check (or willingness to obtain one)
