About the Role:
We are seeking a dedicated and flexible Support Worker (Floater) to join our team. This role is ideal for someone who enjoys variety and is adaptable at working with aged care clients who have physical disabilities, dementia and other limitations. As a floater, you will provide support wherever required, ensuring continuity of care and assisting our clients to maintain their independence and quality of life.
Key Responsibilities:
- Support clients with daily living activities in accordance with their individual care plans.
- Support clients with transportation to access the community for their personal needs like shopping, medical appointments and their social activities.
- Provide home visitations and regular phone calls to clients.
- Maintain accurate work documentation and report any changes in client needs or wellbeing.
- Knowledge of the new Aged Care expectations such as the Code of Conduct, Statement of Rights & SIRs and other requirements.
- Uphold Jubilee Community Service’s values of respect, compassion, and professionalism.
Essential Requirements:
- Certificate IV (or above) in Aged Care, Home & Community Care or Community Services.
- Broad experience working with aged care cliental in home and community care.
- Current First Aid and CPR Certificate.
- Current Working with Children Check (WWCC) and National Police Check (or willingness to obtain).
- Current driver’s license and reliable, insured vehicle.
- Basic - Intermediate Microsoft Word and Excel skills.
- Strong communication and interpersonal skills.
- Flexibility to work at short notice as required.
