Settlement Support and Community Care | Footscray | Maximum Term - Full-time, Maximum Term - Part-time

  • Full-time (or Part-time 4 days) Maximum Term position to 30 June 2026, based at our Footscray, Dallas, Noble Park or Dandenong office
  • This position provides care coordination and health systems’ navigation to employees and key stakeholders associated with the Humanitarian Settlement Program
  • Annual salary is $100,790 + 12% super

About the role:

The Coordinator, Settlement Health serves as a vital link between settlement services, refugee health programs, and the broader healthcare system. This role is focused on enhancing communication and collaboration across these sectors, ensuring that refugees can access health services that are culturally responsive, well-informed, and equipped to meet their unique needs.

To be considered for this role, you will be able to demonstrate the following key selection criteria:

  • Minimum of a Degree (Bachelor) in Nursing or similar field; AND
  • At least 3 years’ experience working as a health practitioner in a community environment
  • Demonstrated ability to support a coordinated approach by working with a range of stakeholders to improve health outcomes as agreed with the client
  • Excellent interpersonal skills including the ability to liaise effectively with a range of internal and external stakeholders, to contribute to and influence plans and decisions at appropriate organisational levels, and to engage with clients and the community to enable service improvement
  • Demonstrated ability to participate in a performance-driven environment, to work autonomously and as part of a fixed and remote team including a willingness to support team members and to work as part of a broader cohesive care team across an organisation and relevant external partnership groups
  • Proficiency in working effectively with interpreters and bicultural workers either face-to-face or via telephone
  • Knowledge of local and regional refugee services and groups and a demonstrated sensitivity to culturally and linguistically diverse (CALD) individuals and communities
  • Knowledge of client-directed care principles and frameworks, and a sound knowledge of community health principles
  • Demonstrated ability to undertake practice based on primary care, preventative care or early intervention healthcare approaches
  • Demonstrated commitment to the principles of diversity, EEO, and participatory work practices, including access and equity for individuals from disadvantaged communities
  • Demonstrated commitment to and use of appropriate ethical standards and behaviours, and to social justice
  • Exceptional written and verbal communication skills, including the ability to present publicly, to negotiate and gain cooperation and support from others, and to advocate effectively on behalf of clientsThe capacity to inspire trust and confidence in others
  • Demonstrated understanding of the principles of quality systems, corporate governance, risk management, and clinical governance
  • Proficiency in evidence-informed project-based work including good planning, implementation and evaluation skills
  • Ability to manage competing priorities
  • Sound demonstrable knowledge and skill in the use of MS Office applications (including Outlook, Edge, Word, Excel, and PowerPoint), and the ability to quickly learn to effectively use relevant Health electronic records systems (for example, VHIMS, TRIM, Trak Care, Titanium)
  • It is a mandatory requirement for the position holder to have and maintain a current Working With Children Check (Employee).
