Northeast Health Wangaratta (NHW) is the major health provider in the Central Hume region with an extensive range of acute, sub-acute, aged care and community services.
The Motor Neurone Disease (MND) Share Care Worker is a regional role in the Hume Region of Victoria and an integral part of the Palliative Care team, whose purpose is to raise the quality of life for people living with MND by:
- Promoting early referral to palliative care services
- Support, development and delivery of education to palliative care services providers
- Provide coordination between palliative care services, MND clients and families in the Hume region.
The MND Shared Care Worker not only provides support to people living with MND, but also to their families, health professionals and community health care providers. As MND progresses, care needs can rapidly change. Each individual will have their own unique range of symptoms and requirements for support. The MND Shared Care Worker assists in navigating these pathways.
The MND SCW works closely with the MND Victoria Advisor and Support Coordinator.
