Northern Disability and Aged Care (NDAAC) is seeking a NDIS Support Coordinator to join our growing team. As an NDIS-registered provider, we are dedicated to delivering person-centered support that enhances independence, well-being, and community inclusion.
Role Overview
This role is ideal for an individual who is committed to advocating for participants, coordinating support services, and fostering community engagement. You will work closely with clients to identify their needs, develop personalized support plans, and ensure they receive the best possible care and resources.
Key Responsibilities:
· Conduct needs assessments to understand client goals and support requirements.
· Facilitate access to NDIS-funded services, healthcare, housing, and social programs.
· Advocate for participants’ rights and ensure they receive equitable support.
· Organize and encourage participation in community events, skills programs, and recreational activities.
· Develop, implement, and monitor individualized support plans, adjusting as needed.
· Offer emergency support and crisis intervention when required.
· Maintain accurate client records in line with NDIS policies and organizational standards.
What We’re Looking For
We are seeking an individual with a strong commitment to community well-being and a passion for empowering individuals through NDIS and aged care support services.
Essential Criteria:
· Diploma or higher qualification in Community Services, Social Work, Disability, or a related field.
· Minimum 1 year of experience in disability, aged care, or NDIS support services.
· Strong knowledge of NDIS processes and participant advocacy.
· Excellent communication and interpersonal skills with the ability to build trust with clients.
· Ability to work independently and collaborate with service providers.
Mandatory Requirements:
· NDIS Worker Screening Check (or willingness to obtain).
· Working with Children Check & Police Clearance.
· Valid Australian Driver’s License & reliable vehicle.
· First Aid and CPR Certification (preferred).

Pay: $78,000.00 – $80,000.00 per year
