NDIS SUPPORT COORDINATOR / RECOVERY COACH

We are looking for an energetic go-getter to take on existing Support Coordination clients, bringing your own expertise, knowledge and network-base to the organisation.
No need for your own registration and audit, you will work under a proven, existing, compliant registration.

Key Responsibilities as a Support Coordinator/Recovery Coach ...
- Assist NDIS participants in implementing their NDIS Plans and coordinating their services and supports.
- Support clients to achieve their goals through strength-based and person-centred planning.
- Complete all necessary administration to the required standard, ensuring full compliance.
- Promote the team to the market and generate interest in support coordination under the NDIS. This may involve delivering presentations to various organisations such as local care coordinators, disability service providers and support groups including participants and nominees.
- Maintain and develop relationships with government bodies (Justice, health, education, employment etc), NFPs and disability associations/groups, and other NDIS Providers
- Take ownership of developing and retaining networks.
- Coordinating NDIS packages and fostering meaningful community connections for clients.
- Develop strong and collaborative working relationships with relevant internal and external stakeholders
- Regularly monitor NDIS plan funding to ensure services are being delivered within the participant's allocated budget.
- Support NDIS participants with recovery planning
- Coach NDIS participants to increase recovery skills and personal capacity, including motivation, strengths, resilience and decision-making
- Supports NDIS participants to engage with the NDIS
About you

With a background in the disability, community, or mental health sectors, you’re recognised for your compassion, resilience, and ability to solve problems effectively. Your excellent communication skills enable you to build strong, trusting connections with participants, their families, and other service providers.
You bring a well-rounded skill set that includes excellent verbal and written communication, along with strong organisational and time management abilities. Your talent for engaging potential clients and converting inquiries into leads demonstrates your strategic acumen. You excel at building and maintaining positive stakeholder relationships, and your ability to work both independently and collaboratively makes you a valuable asset in any team environment.
Requirements:
- Current driver's license and willingness to travel to clients where required
- Detailed working knowledge of the NDIS.
- Allied Health qualifications or a minimum Certificate IV in Allied Health, Disability, Aged Care, or similar
- Experience with psychosocial disability is essential
- Experience or knowledge working in the Community Services sector (Disability, Community Health, Mental Health, Housing, Justice, Child Protection, Psychology, Social Work, Employment Services) would be beneficial
- A minimum of two (2) years' experience as a NDIS Support Coordinator, NDIS LAC, Planner, Human Services or Case Manager is desirable
- Demonstrated ability to be client-focused and work collaboratively with participants, carers, loved ones and stakeholders.
- Established time management, financial budgeting, problem solving and conflict management skills.
- Ability to self-motivate, complete tasks within set time frames, manage caseload and conflicting deadlines and work within a Key Performance Indicator framework.
- Strong written, administration and computer skills for the creation of case notes, reports, correspondence and other documents
