About the Role
We’re seeking a dedicated and empathetic Non-Clinical Needs Assessor to join our large, supportive Aged Care Assessment Service team. You’ll help older people and their carers access the right services by conducting aged care assessments in both community and hospital settings.
Using the Integrated Assessment Tool (IAT), you’ll assess each person’s medical, physical, cognitive, psychosocial, cultural, environmental and social needs, then develop a tailored support plan that enables them to live with dignity and independence.
This is a role where your organisation, empathy and communication skills will directly improve quality of life for older people across the Hunter.
In this role, you’ll:
- Conduct aged care assessments for clients referred via the My Aged Care Contact Centre
- Formulate support plans in collaboration with clients, carers and relevant services
- Accurately document findings and recommendations
- Work both independently and as part of a multidisciplinary team
- Support service access across multiple community and hospital-based locations
About You
You’re a compassionate, detail-oriented professional with a genuine passion for supporting older people. You can juggle competing demands while staying focused on the person in front of you.
You must have:
- Experience working with aged clients in a community or health setting
- Strong time management skills with the ability to prioritise effectively
- Excellent written and verbal communication skills
- The ability to work independently and collaboratively
- Confidence with accurate, timely documentation
