Meaningful Career Helping Young People living in Out of Home Care
Competitive Salary + Benefits
The Role
Join our dedicated team in supporting children, young people, their siblings, foster carers, birth families, and other significant people to achieve safe, stable, and lasting placement outcomes.
As a Permanency Case Manager, you will deliver therapeutic, trauma-informed case management based on evidence-based practice. Your role will ensure that all aspects of a child or young person’s wellbeing are addressed, and that birth families receive the support they need to enhance their overall functioning and wellbeing.
Where possible, permanency planning focuses on family preservation or restoration. When these are not viable, your role will support alternative permanency options such as guardianship, long-term foster care, or adoption.
This is a dynamic role involving complex case management, direct client engagement, and practical support to children, young people, carers, and families. It also requires detailed documentation and reporting. Support is provided in homes, community settings, and office environments, and some work outside of core business hours may be required to respond to the urgent needs of children, young people and families.
