Job Description
Seeking a Project Lead - Multicultural Services Access
Fixed Term 12 month project role commencing July 2025 until 30 June 2026

DPV Health has recently been awarded the “Improving access to primary care for multicultural communities” project on behalf of the North Western Melbourne PHN (NWMPHN). The purpose of the project is to address the needs of multicultural communities within Hume LGA for navigation support and equitable service experience from the primary care system.
The project aims to increase engagement with the healthcare system for multicultural communities leveraging already established networks, work collaboratively with multicultural organisations, local councils and community health providers to disseminate information on primary health services.
This role has specific accountabilities aligned with the project as per the agreement with the NWMPHN. The role will have leadership responsibilities for 2 Community Health navigators, as direct reports.
This project is a 12-month project commencing 01 July 2025 and ending 30 June 2026.

You have experience and knowledge of working with diverse communities and apply a social determinants and strength-based approach.
Your excellent interpersonal and communications skills, as well as your ability to build relationships and work collaboratively, will see you successfully lead this project.
