Altido Consulting Services (Altido) is an approved provider of NDIS (National Disability Insurance Scheme) and Veterans Home Care and offers Allied Health services. Altido was established in the year 2013 and has been present in the industry for over 10 years.

Key Responsibilities:
As a Service Coordinator, you will:
- Support people living with disabilities in achieving their goals and enhance their independence
- Complete the onboarding process for new clients
- Coordinate and schedule support services/activities in accordance with established serviced plans and organisational policies.
- Coordinate and link informal, funded, and mainstream services
- Ensure high-quality, individualised support services
- Foster strong community relationships and partnerships
- Monitor and assess the quality of care provided, identifying areas for improvement, and implementing changes as needed.
- Maintain compliance with WHS and NDIS standards
- Regular Spot Checks must be carried out
- Provide administrative support as needed, including creating service agreements and ensuring alignment between the agreements and actual support shifts
- Be available to cover last-minute shift cancellations when required
- Attend marketing events to promote Altido’s support work and Allied health therapy services
- Conduct cold calls to previous clients to explore their interest in re-engaging with our services or discuss any additional support they may require.
Qualifications and Experience
· Minimum Certificate III in Community Services or equivalent work experience
· Driver's License and comprehensively insured private vehicle.
· Knowledge of the NDIS sector and related issues.
· Proven ability to assess, plan, implement, and evaluate support services.
· Strong interpersonal and communication skills, both written and oral.
· Computer literacy and analytical abilities.
