Social Worker (Acquired Disability) - Full-time Social Work role in our Toowoomba Office
Rehability Australia is seeking an experienced Social Worker to join our Disability & Case Management Team, providing specialist disability and rehabilitation services to individuals living with Acquired Disabilities - Brain Injury (ABI) and Spinal Cord Injury (SCI).

About the Role
As a Social Worker in our Toowoomba team, you will:
- Deliver specialist social work and case management rehabilitation services for clients with ABI and SCI.
- Conduct psychosocial assessments and develop tailored support and rehabilitation plans.
- Liaise with medical professionals, allied health providers, support coordinators, and community services to deliver high-quality outcomes.
- Advocate strongly for clients to access appropriate funding, services, and resources across schemes including NDIS, NIISQ, icare, TAC, DVA, and other insurers.
- Utilise Flowlogic CRM to manage client information and track service delivery effectively.
- Build and maintain local disability sector connections to improve service accessibility for clients.
What We’re Looking For
- Bachelor’s or Master’s degree in Social Work
- Active membership with the Australian Association of Social Workers (AASW).
- Minimum 2–5 years’ direct experience working with people who have sustained Acquired Brain Injury or Spinal Cord Injury.
- Proven skills in social work, case management and rehabilitation planning.
- Strong knowledge of Toowoomba’s disability services sector and established professional networks.
- Ability to work independently while collaborating effectively within a multidisciplinary team.
- Experience using Flowlogic CRM software.
- A valid driver’s licence and reliable vehicle.
