Full-time | Work from home with local travel around Toowoomba, Darling Downs and surrounds
MS Queensland is growing, and we’re looking for a passionate Support Coordinator to join our team! In this full-time, permanent role, you’ll work directly with customers living with Multiple Sclerosis and other progressive neurological conditions, assisting them to access the services, supports, and advice they need to live empowered, fulfilling lives.
You’ll be the bridge between individuals and the broader community sector, guiding customers through the NDIS process, assisting them to implement their plans, and delivering truly person-centred support. Based in your own home office within Toowoomba/ Darling Downs you’ll also meet with customers and stakeholders across the Toowoomba and Darling Downs region.

- Build strong networks across the sector to open doors to new opportunities and ensure customers have real options.
- Grow capacity and confidence by providing guidance and support that helps customers manage their plans and build life skills.
- Drive positive change, collaborate with stakeholders, share feedback, and help shape a culture of respect, inclusion, and continuous improvement.
- Proven Support Coordination or Case Management experience in disability or human services, with a strong foundation in coordinating community-based support and navigating complex care environments.
- Clear and confident communication—able to engage diverse audiences, build trust, and rapport.
- Strong working knowledge of the NDIS and local service networks, with the ability to drive positive outcomes.
- Relationship-focused mindset—skilled in building partnerships, fostering collaboration, and maintaining respectful, person-centred connections.
- Self-driven and adaptable approach, with the initiative to manage time, solve problems, and contribute to a high-performing team culture.
