About the role:
GEM@Home is a geriatrician-led multidisciplinary service, offering care to the older person, with complex medical, functional and or psychosocial needs, in their own home. Community Geriatrics provides short-term case management to prevent hospital admissions, through the delivery of care in the home environment.
Reporting Operationally to the Advanced Nurse Unit Manager, some of your duties for the role will include:
- Individual case management, care planning, and liaison for older patients in the community
- Utilise independent decision making to providing direct, expert clinical care, selecting, and implementing different therapeutic interventions
- Effective complex discharge planning / hospital avoidance through the provision of education, equipment and referral including client/program evaluation
- Linking clients and carers with relevant community resources and services
- Communicating with clients' family, carers, and other service providers
- Contributing to the development of an integrated team approach and culture which is highly responsive to client needs
About you:
- RN3: Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate,
- Registered Nurse and or Midwife with at least 3 years post-registration experience,
- Demonstrated competence in geriatrics and community Nursing practice in accordance with the relevant standards,
- AHP3: Must hold an appropriate degree level qualification and membership (or eligibility for membership) with the relevant professional association, and/or registration with the relevant Board – Physiotherapy, Occupational Therapy, Speech Pathology, Dietetics, Psychology, Social Work or Exercise Physiology,
- Demonstrated ability to assume responsibility for the provision of clinical allied health services to a designated area as a member of an interdisciplinary team,
- Demonstrated ability to undertake assessment and intervention of clients with a variety of disorders and disabilities, including management of complex cases, particularly those with cognitive impairment or a diagnosis of dementia,
- Demonstrated experience in clinical management and leadership within a discrete unit, including program evaluation and service development.
