About the Opportunity
The Case Manager Lead works within Circuit Breaker Refuge to provide structured support to young people who are accommodated at Frontyard. The role plays a key critical function in oversight of the team led case management through implementing care-plan meetings, facilitate and lead care coordination meetings with internal and external providers, and participate in service coordination to promote a team case management approach.
About you
We are looking for someone who can:
Facilitate and support a team case management approach with our multi-disciplinary team including Youth Refuge Workers, Senior Youth Alcohol and Other Drugs Recovery Lead, and Senior Practice Leader to young people through a healing-oriented framework responses to young people who are homeless or at risk of homelessness.
Support young people at Circuit Breaker and in the community in accordance with their care plan
Develop and maintain case plans constructed alongside young people that create positive and sustainable pathways out of homelessness.
Work with our multi-disciplinary team to facilitate access to all specialist support services, including physical and mental health, AOD and therapeutic interventions.
Maintain an up-to-date knowledge of current youth homelessness trends and statewide youth support programs available.

Essential
A bachelor qualification in youth work/social work or a related tertiary qualification
Demonstrated experience working within a case management framework supporting people in accommodation (at least two years)
Understanding of assertive outreach support and interventions, including settlement support with a sustaining tenancies practice lens
Understanding or experience working with young people at risk or experiencing homelessness, including knowledge of healing-oriented approaches
Strong passion for working with young people with the ability to engage young people actively and assertively with complex needs
An extensive understanding of the homelessness service system with knowledge of patterns, trends, systemic issues and principles in working with at risk young people
Knowledge of the legislative requirements when working with at risk young people.
An understanding of the Child Safe Standards
Excellent communication, delegation, problem-solving and advocacy skills

Provide First Aid certificate
Maintain accurate files, case notes and databases using relevant platforms and systems
Availability to work on-site and in the community Monday to Friday standard working hours.
Screening Criteria
National Police check
Working with Children check and NDIS Worker Screening check or ability to obtain
Valid Victorian Drivers license
Right to Work in Australia
