About the program
Wesley Home Care offers elderly Australians a range of services designed to enhance their wellbeing and promote independence. Care and services to support and assist older people who wish to remain living in their own homes is provided through the Commonwealth Home Support Program and Home Care Packages. From November 1st, 2025, the Support at Home program will replace the Home Care Packages Program. These major programs are a key focus of our service delivery in home care.
About the role
The Clinical Care Advisor provides holistic care by collaborating with Wesley Mission consumers and their representatives, to build an individual care plan and budget under the current model of Home Care packages, and when transitioned, to the model of Support at Home.
Respectful care and support are achieved for the consumer through the identification of consumer goals of individual and clinical importance. This enables safe and meaningful opportunities for consumers in their home and in the community.
The Clinical Care Advisor drives the process of consumer evaluation and re-evaluation on a continuous four monthly cycle enabling the correct direction of care and support to be followed. This assessment cycle achieves the delivery of safe and quality care and the meeting of desired consumer outcomes.
Selection Criteria
Essential criteria
- current registration as a Registered Nurse Division 1 under the Australian Health Practitioner Regulation Agency (AHPRA)
- minimum 3 years working in a clinical setting as a Registered Nurse
- an understanding of the Aged Care Quality Standards
- knowledge of My Aged Care funding and Aged Care services
- Class C drivers licence (minimum P2 license to drive a Wesley Mission car)
- satisfactory Criminal History Check and Working with Children Check.
Desirable criteria
- experience working as a Community Nurse
- Case Management experience
Remuneration
In accordance with the Social, Community, Home Care and Disability Services Industry Award 2010, Level 5 + superannuation + salary packaging + fully maintained vehicle.
