About the Role
- Triage and provide comprehensive assessments and develop tailored treatment plans for clients in Residential Aged Care
- Deliver consultations via telehealth
- Develop personalised nutrition/ diet plans
- Collaborate with facility staff, families, and other health professionals
- Weight management and unplanned weight loss
- Monitoring, reporting, reviewing, compliance audits and following up with catering team
- Any additional dietetic related requested by facility
- Ensure compliance with aged care standards and Amplar Health policies
- Tertiary qualifications in Dietetics
- Accredited Practising Dietitian (APD) with Dietitians Australia
- Experience in aged care (desirable)
- Strong communication and interpersonal skills
- Ability to work autonomously and manage your own schedule
