About the Role
The NDIS Care Coordinator is position designed to support participants in navigating their NDIS plans, connecting with appropriate services, achieving their goals and building their capacity. This role provides a pathway to becoming a NDIS Support Coordinator, offering training and mentorship to build foundational knowledge of the NDIS and develop skills in person-centred culturally safe service delivery.
Duties
- Assist participants in understanding their NDIS plans and available supports.
- Assist in linking participants with appropriate service providers and community resources.
- Ensure compliance with organisational policies and NDIS guidelines.
- Participate in meetings with participants, families and other stakeholders.
- Develop an understanding of person-centred and culturally safe approaches.
- Ensure services provided align with the needs of the Aboriginal and Torres Strait Islander community.
- Participate in training and mentoring opportunities to build skills for future career progression.

Required
- Certificate III or higher qualification in Health Care, Social Assistance, Community Services, Administration, or related fields or minimum of one year of experience in disability, aged care, or a similar field.
- Strong communication and interpersonal skills.
- Organisational and time management skills.
- Ability to work independently and as part of a team.
- Commitment to culturally safe and trauma-informed service delivery.
- Sound knowledge of Microsoft Suite applications.
- National Police Certificate.
- Working with Children's Card (Blue Card) or be eligible to apply.
- Disability Worker Screening (Yellow Card) or be eligible to apply.
- Willing to travel to our other locations.
- Previous experience working with Aboriginal & Torres Strait Islander clients.
- Experience with the National Disability Insurance Scheme (NDIS).
- Case management and coordination experience using a client information system.
- Demonstrated ability to meet targets and performance outcomes.
