Inspired by the life and story of Alice, the niece of our founders, we believe that everyone deserves the chance to be able to achieve their goals and live a full and productive life. We aim to support our participants with the highest quality, individualised support, guided by the goals they have outlined in their NDIS plan.

About the NDIS:
The NDIS is the largest social reform in Australia for decades. It is changing the lives of people living with a disability by giving them choice and control over the supports they access. Support Coordination plays an important role in helping some of the most marginalised members of society realize the full potential of the NDIS.
Locations
We are looking at hiring an experienced Support Coordinator equipped with managing complex caseloads to join our growing team in Thomastown.
The Role
A Support Coordinator is responsible for supporting the National Disability Insurance Scheme (NDIS) participants to activate, interpret and implement their NDIS Plans. As a Support Coordinator, you will work with scheme participants between the ages of 0 – 65 with varying complexity levels. You will coordinate service providers and supports, liaise with government agencies, obtain quotes for services, organise service agreements and monitor participants plans and their budget utilisation. You will also provide regular Support Coordination reports to the NDIA for each Participant you support.

Qualifications & Experience:
- Minimum Certificate IV in Disability or related field
- Experience/ qualifications within the Hospital/Healthcare system is desirable
- Demonstrated knowledge and experience of NDIS
- Experience in working with people with complex support and service requirements.
- Previous Support Coordination or Case Management experience
- Current First Aid and CPR Certificate (or willing to obtain)
- Australian Driver's license and vehicle
- NDIS Worker Screening check (or willing to obtain)
- Working with Children Check
Tasks & Responsibilities:
- As a Support Coordinator, your key responsibilities will include:
- Providing high-quality Level 2 Support Coordination to NDIS participants and providing assistance to their families/informal supports in navigating the NDIS
- Building strong relationships with participants, families, providers, and stakeholders an ensuring responsiveness and clear communication is maintained
- Promoting services through existing networks and local engagement
- Assisting new participants with smooth transitions into services
- Maintaining accurate, up to date records, reports, and billing using NDIS platforms
- Collaborating with the team to ensure excellent participant outcomes
- Liaising with key stakeholders, including NDIS providers, agents, community venues, and allied health teams
- Monitoring plan outcomes and expenditure to stay within budget
- Independently managing a varied caseload of participants, ensuring quality care and meeting objectives
- Conducting initial assessments and referring participants to relevant services with a focus on recovery
- Ensuring participants are progressing toward their goals and documenting progress in reports
- Ensure integrity, honesty and transparency is always present, and strong alignment and of the NDIS Code of Conduct is maintained.
