Pregnancy Connect Care Coordinator

About the role
This is a unique and exciting opportunity to drive meaningful change for pregnant women experiencing risks and vulnerabilities within our Local Health District (LHD).
In this newly established role, you will work collaboratively across the district with a range of stakeholders to strengthen and improve the systems and processes that ensure women, and their families receive the right care, at the right time, and in the right place.
We are seeking a dynamic and experienced clinician with strong clinical and leadership capabilities. You will demonstrate flexibility, initiative, and the ability to work both independently and as part of a cohesive team. In this role, you will work closely with the District Obstetric Advisor, colleagues in the Tiered Perinatal Network, and the broader NSW Maternity and Newborn Network.
This is an exciting opportunity for a motivated professional to shape a new role and make a lasting impact on perinatal care across the district.
