Position Summary
The Case Manager plays a critical dual role in supporting both NDIS participants and internal staff. This position is responsible for coordinating and implementing person-centred supports aligned with participants’ NDIS plans, as well as supporting and supervising staff to ensure high-quality, compliant service delivery. The Case Manager ensures that all services meet the NDIS Practice Standards, reflect best practices in disability support, and align with each participant’s goals and needs.
Key Responsibilities:
Participant-Focused Responsibilities
- Conduct intake, needs assessments, and risk assessments for new participants.
- Assist participants and their families to understand and implement their NDIS plans.
- Develop and maintain support plans, service agreements, and risk management strategies.
- Coordinate support services and liaise with external providers, health professionals, and community organisations.
- Monitor participant progress toward goals, adjust services as needed, and prepare for plan reviews.
- Advocate for participant rights and access to services.
- Respond promptly to incidents, complaints, and changes in participant needs.
Staff-Focused Responsibilities
- Supervise and support frontline staff to ensure quality service delivery.
- Provide case-specific direction and guidance to support workers and allied health staff.
- Facilitate team meetings, case conferences, and staff debriefings as needed.
- Monitor documentation quality, including case notes, progress reports, and incident reporting.
- Identify training needs and support staff professional development.
- Ensure compliance with NDIS Quality and Safeguards Commission requirements and organisational policies.
Essential:
- Tertiary qualifications in Social Work, Psychology, Disability, Community Services, or a related field.
- Proven experience working with NDIS participants and understanding of NDIS legislation and service delivery frameworks.
- Strong case management experience, including assessment, planning, and review.
- Demonstrated ability to supervise, mentor, and support staff teams.
- Excellent communication, organisational, and interpersonal skills.
- Sound understanding of person-centred and trauma-informed practices.
- High-level documentation and IT skills.
Desirable:
- Experience in a leadership or coordination role within the disability or community services sector.
- Lived experience or deep understanding of disability and/or mental health.

- NDIS Worker Screening Clearance
- Current Driver’s Licence and access to a reliable vehicle
- Current First Aid & CPR certification
