We’re on the lookout for a passionate NDIS Support Coordinator to help participants navigate their plans, connect with providers, and achieve their personal goals.
What You’ll Do:
- Coordinate supports and services aligned with participant goals
- Build capacity and independence
- Collaborate with families, providers, and the NDIA
- Manage a diverse caseload with flexibility and purpose
- Day Program/Group Home activities/ School Holidays
- Rostering
What You’ll Bring:
- Experience in Support Coordination or Case Management
- Relevant qualifications (Cert IV or Bachelor in Disabilities or Individual Support or Community Service)
- Knowledge of the NDIS Compliance
- Excellent communication and organisational skills
- NDIS Worker Check and WWCC (Mandatory)
- Driver’s licence
